que la p\u00e9nurie mondiale de personnel de sant\u00e9 atteindra 14 millions de personnes d&#8217;ici 2030. Pour rem\u00e9dier \u00e0 cette p\u00e9nurie, l&#8217;Organisation mondiale de la sant\u00e9 a encourag\u00e9 l&#8217;int\u00e9gration des professionnels de sant\u00e9 form\u00e9s \u00e0 l&#8217;\u00e9tranger (IEHP) dans les syst\u00e8mes de sant\u00e9. Les IEHP sont des travailleurs qualifi\u00e9s qui ont suivi leur formation, leur apprentissage et obtenu leur accr\u00e9ditation dans un pays autre que celui dans lequel ils souhaitent exercer. Les IEHP sont essentiels pour rem\u00e9dier \u00e0 la p\u00e9nurie de main-d&#8217;\u0153uvre et am\u00e9liorer la qualit\u00e9 des soins. Au Canada, les IEHP repr\u00e9sentent 13 % des personnes ayant suivi des \u00e9tudes postsecondaires dans le domaine de la sant\u00e9, mais ces professionnels sont sous-repr\u00e9sent\u00e9s et sous-utilis\u00e9s.&nbsp;<\/p>\n\n\n\n<p>Les IEHP sont confront\u00e9s \u00e0 un large \u00e9ventail d&#8217;obstacles soci\u00e9taux, organisationnels et individuels qui entravent leur capacit\u00e9 \u00e0 poursuivre leur carri\u00e8re au Canada. Il s&#8217;agit notamment du syst\u00e8me de sant\u00e9 d\u00e9centralis\u00e9 du Canada, qui entra\u00eene des incoh\u00e9rences dans la reconnaissance des titres de comp\u00e9tences et les exigences en mati\u00e8re d&#8217;autorisation d&#8217;exercer d&#8217;une province \u00e0 l&#8217;autre et d&#8217;un territoire \u00e0 l&#8217;autre. Au niveau organisationnel, les r\u00e9glementations suppl\u00e9mentaires en mati\u00e8re d&#8217;autorisation d&#8217;exercer, les possibilit\u00e9s limit\u00e9es de r\u00e9sidence ou de stages cliniques et la demande persistante d&#8217;\u00ab exp\u00e9rience canadienne \u00bb cr\u00e9ent des obstacles importants. Les pratiques insuffisantes des organisations en mati\u00e8re d&#8217;\u00e9quit\u00e9, de diversit\u00e9 et d&#8217;inclusion (\u00c9DI) et les exp\u00e9riences de racisme et de discrimination v\u00e9cues par les IEHP ont \u00e9galement une incidence sur le recrutement, la promotion et la fid\u00e9lisation. Au niveau individuel, les obstacles comprennent les difficult\u00e9s li\u00e9es \u00e0 la ma\u00eetrise de la langue, l&#8217;adaptation aux normes socioculturelles et professionnelles canadiennes, ainsi que la tension personnelle et \u00e9motionnelle li\u00e9e \u00e0 la migration et \u00e0 l&#8217;interruption de carri\u00e8re.<\/p>\n\n\n\n<p class=\"has-black-color has-text-color\">Ce rapport fait r\u00e9f\u00e9rence \u00e0 des exemples de pratiques exemplaires canadiennes et internationales afin de montrer comment les IEHP peuvent \u00eatre soutenus au sein du personnel de sant\u00e9 canadien. Les pays qui ont r\u00e9alis\u00e9 des progr\u00e8s importants en mati\u00e8re d&#8217;int\u00e9gration de l&#8217;IEHP, tels que l&#8217;Australie, l&#8217;Irlande et le Royaume-Uni, ont adopt\u00e9 des strat\u00e9gies innovantes pour rationaliser la reconnaissance des dipl\u00f4mes et cr\u00e9er des parcours structur\u00e9s pour l&#8217;int\u00e9gration de la main-d&#8217;\u0153uvre. Par exemple, en Australie, gr\u00e2ce \u00e0 un leadership centralis\u00e9 fort, \u00e0 une s\u00e9lection ax\u00e9e sur les employeurs, \u00e0 une migration ax\u00e9e sur la demande, ainsi qu&#8217;\u00e0 des investissements dans des programmes de transition et de reconnaissance pr\u00e9coce des dipl\u00f4mes, moins de 5 % des infirmi\u00e8res et 10 % des m\u00e9decins form\u00e9s \u00e0 l&#8217;\u00e9tranger gagnent moins que le revenu m\u00e9dian. Cela contraste avec le Canada, o\u00f9 70 % des m\u00e9decins form\u00e9s \u00e0 l&#8217;\u00e9tranger arriv\u00e9s entre 2009 et 2016 gagnaient moins de la moiti\u00e9 du revenu m\u00e9dian de leurs homologues n\u00e9s au Canada.<\/p>\n\n\n\n<p>Au Canada, un nombre croissant d&#8217;innovations organisationnelles et r\u00e9glementaires visent \u00e0 rem\u00e9dier \u00e0 l&#8217;opacit\u00e9, \u00e0 la fragmentation et \u00e0 la complexit\u00e9 du processus d&#8217;obtention d&#8217;un permis d&#8217;exercice, notamment par la pr\u00e9paration avant l&#8217;arriv\u00e9e, les accords de reconnaissance mutuelle, la reconnaissance des titres de comp\u00e9tences fond\u00e9e sur les comp\u00e9tences, les programmes de transition mis en place par les employeurs et les \u00e9valuations de l&#8217;aptitude \u00e0 exercer qui permettent aux IEHP d&#8217;acqu\u00e9rir des comp\u00e9tences et de d\u00e9montrer leur aptitude \u00e0 occuper un emploi. Les employeurs du secteur de la sant\u00e9 ont \u00e9galement pris des mesures importantes, notamment en favorisant la diversit\u00e9 au sein de la direction et en adoptant des politiques inclusives, en mettant en \u0153uvre des strat\u00e9gies en mati\u00e8re de ressources humaines telles que le recrutement cibl\u00e9, le mentorat et l&#8217;orientation prolong\u00e9e, et en instaurant une culture d&#8217;entreprise inclusive qui renforce la fid\u00e9lisation et l&#8217;engagement des employ\u00e9s.<\/p>\n\n\n\n<p class=\"has-black-color has-text-color\">Sur la base de ces recherches, le rapport recommande de renforcer et d&#8217;\u00e9tendre les initiatives gouvernementales visant \u00e0 rationaliser les conditions d&#8217;obtention des licences et \u00e0 affiner les approches cibl\u00e9es en mati\u00e8re d&#8217;immigration, les organismes de r\u00e9glementation se concentrant sur les comp\u00e9tences d\u00e9montr\u00e9es plut\u00f4t que sur le lieu de formation. Les employeurs sont encourag\u00e9s \u00e0 adopter des m\u00e9thodes d&#8217;int\u00e9gration globales et \u00e0 mettre en place des strat\u00e9gies \u00c9DI efficaces afin de favoriser une culture d&#8217;entreprise inclusive qui reconna\u00eet les contributions des IEHP.<\/p>\n\n\n\n<div class=\"wp-block-group key-insights-grey has-background is-layout-constrained wp-block-group-is-layout-constrained\" style=\"background-color:#0000000d\">\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<h5 class=\"wp-block-heading\"><strong><strong>Perspectives cl\u00e9s<\/strong><\/strong><\/h5>\n\n\n\n<p class=\"is-style-default is-flex\"><img decoding=\"async\" class=\"wp-image-243230\" style=\"width: 40px;margin-right: 16px;\" src=\"https:\/\/fsc-ccf.ca\/wp-content\/uploads\/2024\/07\/No.svg\" alt=\"\">Alors que 76 % des IEHP ont un emploi, seuls 58 % occupent des postes li\u00e9s aux soins de sant\u00e9, beaucoup d&#8217;entre eux occupant des postes qui ne correspondent pas \u00e0 leur expertise.<\/p>\n\n\n\n<p class=\"is-flex\"><img decoding=\"async\" class=\"wp-image-243230\" style=\"width: 40px; margin-right: 16px;\" src=\"https:\/\/fsc-ccf.ca\/wp-content\/uploads\/2024\/07\/No-1.svg\" alt=\"\">Les IEHP sont confront\u00e9s \u00e0 un ensemble complexe d&#8217;obstacles qui doivent \u00eatre surmont\u00e9s par tous les acteurs du syst\u00e8me de sant\u00e9, y compris les gouvernements, les organismes de r\u00e9glementation et les employeurs. Parmi les obstacles importants, citons les exigences incoh\u00e9rentes en mati\u00e8re d&#8217;obtention de permis d&#8217;exercice, la discrimination syst\u00e9mique \u00e0 l&#8217;\u00e9gard des IEHP qui affecte leur exp\u00e9rience professionnelle, et l&#8217;absence de programmes ax\u00e9s sur l&#8217;\u00c9DI pour aider les IEHP \u00e0 s&#8217;int\u00e9grer dans le milieu de travail canadien.<\/p>\n\n\n\n<p class=\"is-flex\"><img decoding=\"async\" class=\"wp-image-243230\" style=\"width: 40px; margin-right: 16px;\" src=\"https:\/\/fsc-ccf.ca\/wp-content\/uploads\/2024\/07\/No-2.svg\" alt=\"\">Les pratiques exemplaires canadiennes et internationales fournissent des exemples convaincants de la mani\u00e8re dont les IEHP peuvent s&#8217;int\u00e9grer plus harmonieusement dans le syst\u00e8me de sant\u00e9. Les politiques internationales de pays comme l&#8217;Australie, l&#8217;Irlande et le Royaume-Uni sugg\u00e8rent des changements structurels pour valider les dipl\u00f4mes et les qualifications des IEHP, tandis que les meilleures pratiques organisationnelles des employeurs du secteur de la sant\u00e9 sugg\u00e8rent un besoin urgent d&#8217;int\u00e9grer l&#8217;\u00c9DI dans les politiques et pratiques organisationnelles.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n\n\n\n<div id=\"the-issue\" class=\"wp-block-ob-blocks-anchor-point\"><\/div>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column gradient-top purple is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:100%\">\n<h3 class=\"wp-block-heading has-black-color has-text-color\" id=\"TheChallenge\"><img decoding=\"async\" class=\"wp-image-240812\" style=\"width: 12px;height: 24px;margin-right: 10px;position: relative; top: -2px;\" src=\"https:\/\/fsc-ccf.ca\/wp-content\/uploads\/2024\/07\/Vector-2.svg\" alt=\"\"> L\u2019enjeu<\/h3>\n\n\n\n<div class=\"wp-block-group flex-print is-layout-constrained wp-block-group-is-layout-constrained\">\n<div class=\"wp-block-group is-layout-constrained wp-block-group-is-layout-constrained\">\n<p>Le monde est confront\u00e9 \u00e0 une p\u00e9nurie mondiale de personnel de sant\u00e9, qui devrait atteindre 14 millions de personnes d&#8217;ici 2030 selon l&#8217;Organisation mondiale de la sant\u00e9. Une partie de la strat\u00e9gie mondiale de l&#8217;Organisation mondiale de la sant\u00e9 en mati\u00e8re de ressources humaines pour la sant\u00e9 consiste \u00e0 int\u00e9grer des strat\u00e9gies efficaces de planification des ressources humaines dans le domaine de la sant\u00e9, notamment en tirant parti du potentiel des IEHP.&nbsp;<\/p>\n\n\n\n<p>Malgr\u00e9 la p\u00e9nurie de main-d&#8217;\u0153uvre dans le secteur de la sant\u00e9 au Canada, les IEHP se heurtent \u00e0 des obstacles importants qui emp\u00eachent leur int\u00e9gration. La discrimination syst\u00e9mique demeure un obstacle majeur, les membres du personnel enseignant et de la direction issus des communaut\u00e9s noires et racialis\u00e9es \u00e9tant victimes d&#8217;in\u00e9galit\u00e9s en mati\u00e8re d&#8217;emploi et d&#8217;avancement. Il s&#8217;agit d&#8217;un probl\u00e8me important \u00e9tant donn\u00e9 que la plupart des IEHP (80,6 %) sont issus de minorit\u00e9s ethniques, les groupes ethniques les plus importants \u00e9tant les Philippins (25,5 %) et les Sud-Asiatiques (21,8 %), suivis des populations chinoises, noires, arabes<\/p>\n\n\n\n<p>et d&#8217;Asie occidentale.&nbsp;<\/p>\n\n\n\n<p>Le Canada se classe au septi\u00e8me rang parmi les pays membres de l&#8217;Organisation de coop\u00e9ration et de d\u00e9veloppement \u00e9conomiques (OCDE) pour l&#8217;int\u00e9gration de l&#8217;IEHP. Les m\u00e9decins form\u00e9s \u00e0 l&#8217;\u00e9tranger au Canada repr\u00e9sentaient 24 % des m\u00e9decins agr\u00e9\u00e9s en 2021, contre une moyenne de 18,9 % dans les pays de l&#8217;OCDE. Des progr\u00e8s ont \u00e9t\u00e9 r\u00e9alis\u00e9s dans le recrutement d&#8217;infirmi\u00e8res form\u00e9es \u00e0 l&#8217;\u00e9tranger, mais la proportion de celles-ci n&#8217;est que de 8,8 % au Canada, soit l\u00e9g\u00e8rement au-dessus de la moyenne de l&#8217;OCDE, qui est de 8,7 %.<\/p>\n\n\n\n<p>Le Canada est en retard par rapport \u00e0 d&#8217;autres pays tant en mati\u00e8re d&#8217;emploi des IEHP que de facilitation de leur acc\u00e8s \u00e0 l&#8217;emploi. Par exemple, en Australie, 10 % des titulaires d&#8217;un dipl\u00f4me de m\u00e9decine obtenu \u00e0 l&#8217;\u00e9tranger gagnent moins que le revenu m\u00e9dian. Comparons cela au Canada, o\u00f9 70 % des IEHP gagnent moins de la moiti\u00e9 du revenu m\u00e9dian. Tout comme l&#8217;Australie, l&#8217;Irlande est un pays pionnier dans l&#8217;attraction et l&#8217;emploi de dipl\u00f4m\u00e9s internationaux en m\u00e9decine, qui repr\u00e9sentent plus de 40 % des m\u00e9decins du pays. Le succ\u00e8s de l&#8217;Irlande d\u00e9coule de politiques et de pratiques strat\u00e9giques, notamment des campagnes de recrutement internationales et des processus simplifi\u00e9s d&#8217;octroi de licences.&nbsp;<\/p>\n\n\n\n<p>Ce rapport souligne que le Canada n&#8217;exploite pas encore pleinement les comp\u00e9tences des IEHP malgr\u00e9 les p\u00e9nuries urgentes en mati\u00e8re de soins de sant\u00e9. Cependant, au sein de cette population, seuls 58 % des personnes ayant un emploi travaillent dans le secteur de la sant\u00e9.&nbsp;<\/p>\n\n\n\n<p>Au Canada, les IEHP sont confront\u00e9s \u00e0 un r\u00e9seau complexe d&#8217;obstacles soci\u00e9taux, organisationnels et individuels \u00e0 l&#8217;int\u00e9gration. Les obstacles soci\u00e9taux comprennent les politiques d&#8217;immigration qui affectent les parcours professionnels des IEHP. De plus, les r\u00e9glementations en mati\u00e8re d&#8217;autorisation d&#8217;exercer sont in\u00e9gales en raison du syst\u00e8me de sant\u00e9 d\u00e9centralis\u00e9 du Canada, ce qui rend l&#8217;int\u00e9gration beaucoup plus difficile. Au niveau organisationnel, l&#8217;\u00e9valuation par le Canada des titres de comp\u00e9tences internationaux des IEHP manque de coh\u00e9rence, et les programmes de transition ne comportent pas les \u00e9l\u00e9ments n\u00e9cessaires pour garantir l&#8217;obtention d&#8217;un permis d&#8217;exercice. Les diff\u00e9rentes professions de sant\u00e9 ont \u00e9galement leurs propres obstacles. Par exemple, les m\u00e9decins, le personnel infirmier et les pharmaciens sont tous confront\u00e9s \u00e0 diff\u00e9rents obstacles dans les processus de recertification et d&#8217;octroi de licence, ainsi que dans les possibilit\u00e9s de r\u00e9sidence et de placement. La discrimination syst\u00e9mique dans les milieux de travail canadiens pr\u00e9sente des obstacles, notamment l&#8217;insistance des employeurs \u00e0 exiger une exp\u00e9rience canadienne lors du processus d&#8217;embauche et les comportements discriminatoires envers les IEHP dans le milieu de travail. Pour les IEHP au niveau individuel, des obstacles tels que la ma\u00eetrise de la langue, l&#8217;adaptation culturelle et les pertes personnelles et \u00e9motionnelles constituent tous des d\u00e9fis importants pour l&#8217;int\u00e9gration.&nbsp;<\/p>\n\n\n\n<p>Les exemples canadiens et internationaux d&#8217;int\u00e9gration r\u00e9ussie constituent des pratiques exemplaires pour l&#8217;int\u00e9gration de l&#8217;IEHP. Par exemple, l&#8217;Irlande a simplifi\u00e9 ses r\u00e9glementations en mati\u00e8re d&#8217;octroi de licences, tandis que le Royaume-Uni a conclu des accords bilat\u00e9raux avec plusieurs pays afin de recruter des IEHP. Au niveau organisationnel, des innovations r\u00e9glementaires, telles que la pr\u00e9paration pr\u00e9alable \u00e0 l&#8217;arriv\u00e9e, les accords de reconnaissance mutuelle et les cat\u00e9gories de licences supervis\u00e9es, ont rationalis\u00e9 les processus d&#8217;octroi de licences, et l&#8217;int\u00e9gration de l&#8217;\u00c9DI dans les lieux de travail du secteur de la sant\u00e9 a permis de cibler le recrutement et la fid\u00e9lisation des IEHP. Non seulement les IEHP peuvent renforcer le syst\u00e8me de sant\u00e9 canadien, mais ils peuvent \u00e9galement offrir des soins plus inclusifs qui r\u00e9pondent aux besoins de populations diverses.&nbsp;<\/p>\n\n\n\n<p>Au niveau soci\u00e9tal, les gouvernements doivent adopter une double perspective : garantir la qualit\u00e9 et l&#8217;accessibilit\u00e9 des soins de sant\u00e9 tout en supprimant les obstacles d\u00e9raisonnables pour les IEHP, afin de les prot\u00e9ger contre toute exploitation. Les exigences en mati\u00e8re d&#8217;autorisation d&#8217;exercer devraient \u00eatre harmonis\u00e9es dans toutes les provinces et tous les territoires afin de promouvoir l&#8217;uniformit\u00e9, l&#8217;\u00e9quit\u00e9 et la transparence dans la reconnaissance des titres de comp\u00e9tences internationaux. Le gouvernement f\u00e9d\u00e9ral devrait \u00e9galement soutenir les programmes de transition, les possibilit\u00e9s d&#8217;apprentissage int\u00e9gr\u00e9 au travail et les \u00e9valuations ax\u00e9es sur la pratique qui donnent des r\u00e9sultats tangibles. En outre, une strat\u00e9gie nationale utilisant des donn\u00e9es d\u00e9sagr\u00e9g\u00e9es devrait \u00eatre \u00e9labor\u00e9e afin de cerner les lacunes syst\u00e9miques et d&#8217;\u00e9clairer l&#8217;\u00e9laboration de programmes et de politiques plus adapt\u00e9s.<\/p>\n\n\n\n<p>Les interventions au niveau organisationnel devraient cibler deux domaines principaux : l&#8217;octroi de licences et l&#8217;am\u00e9lioration de l&#8217;\u00c9DI parmi les employeurs du secteur de la sant\u00e9.
