500&nbsp;000 Canadiennes et Canadiens ont une forme de d\u00e9mence, et ce chiffre devrait pratiquement tripler d\u2019ici \u00e0 2050. Or, les pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne, qui fournissent des soins formels directs aux personnes atteintes de d\u00e9mence, sont souvent insuffisamment form\u00e9s pour r\u00e9pondre aux troubles de communication complexes et aux comportements r\u00e9actifs de ces personnes.<\/p>\n\n\n\n<p>Le projet \u00ab&nbsp;Be EPIC&nbsp;: formation sur la d\u00e9mence \u00e0 l\u2019intention des travailleurs en milieu de carri\u00e8re&nbsp;\u00bb, men\u00e9 \u00e0 l\u2019Universit\u00e9 Western, avait pour but de combler ces lacunes critiques en mettant \u00e0 profit la technologie de r\u00e9alit\u00e9 virtuelle pour dispenser une formation en communication ax\u00e9e sur la personne aux pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne qui interviennent dans des foyers de soins de longue dur\u00e9e et \u00e0 domicile.<\/p>\n\n\n\n<p class=\"has-black-color has-text-color\">Les r\u00e9sultats du projet ont \u00e9t\u00e9 significatifs. En tout, 52&nbsp;personnes, dont 44&nbsp;pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne et huit&nbsp;responsables, ont suivi la formation propos\u00e9e en r\u00e9alit\u00e9 virtuelle. La formation s\u2019est av\u00e9r\u00e9e tr\u00e8s efficace, 98&nbsp;% des pr\u00e9pos\u00e9s l\u2019ayant jug\u00e9e utile. Malgr\u00e9 les difficult\u00e9s de recrutement dues \u00e0 la crise des ressources humaines en sant\u00e9, le projet a atteint 92&nbsp;% de l\u2019objectif fix\u00e9 en mati\u00e8re de recrutement.<\/p>\n\n\n\n<div class=\"wp-block-group key-insights-grey has-background is-layout-constrained wp-block-group-is-layout-constrained\" style=\"background-color:#0000000d\">\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column is-layout-flow wp-block-column-is-layout-flow\">\n<h5 class=\"wp-block-heading\"><strong>Perspectives Cl\u00e9s<\/strong><\/h5>\n\n\n\n<p class=\"is-style-default is-flex\"><img decoding=\"async\" class=\"wp-image-243230\" style=\"width: 40px;margin-right: 16px\" src=\"https:\/\/fsc-ccf.ca\/wp-content\/uploads\/2024\/07\/No.svg\" alt=\"\">Les pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne et les responsables se sont dits tr\u00e8s satisfaits de la formation en raison de la transf\u00e9rabilit\u00e9 du contenu, de l\u2019accent mis sur la communication ax\u00e9e sur la personne et de la conception innovante des simulations en r\u00e9alit\u00e9 virtuelle. Les pr\u00e9pos\u00e9s ont indiqu\u00e9 qu\u2019ils se sentaient plus confiants au moment de fournir des soins aux personnes atteintes de d\u00e9mence et qu\u2019ils avaient \u00e9t\u00e9 amen\u00e9s \u00e0 r\u00e9fl\u00e9chir \u00e0 leurs approches de communication.<\/p>\n\n\n\n<p class=\"is-flex\"><img decoding=\"async\" class=\"wp-image-243230\" style=\"width: 40px;margin-right: 16px\" src=\"https:\/\/fsc-ccf.ca\/wp-content\/uploads\/2024\/07\/No-1.svg\" alt=\"\">En tout, 98&nbsp;% des pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne ont jug\u00e9 la formation utile et 79&nbsp;% avaient d\u00e9j\u00e0 recommand\u00e9 la formation Be EPIC-VR avant la fin du projet. Cinq mois apr\u00e8s la formation, Be EPIC-VR reste le mod\u00e8le de formation le plus pl\u00e9biscit\u00e9 par rapport aux autres modalit\u00e9s.<\/p>\n\n\n\n<p class=\"is-flex\"><img decoding=\"async\" class=\"wp-image-243230\" style=\"width: 40px;margin-right: 16px\" src=\"https:\/\/fsc-ccf.ca\/wp-content\/uploads\/2024\/07\/No-2.svg\" alt=\"\">La formation a permis d\u2019am\u00e9liorer les pratiques de soins, de renforcer la s\u00e9curit\u00e9 du personnel et des r\u00e9sidents et de faire conna\u00eetre des strat\u00e9gies de communication visant \u00e0 pr\u00e9venir et \u00e0 d\u00e9samorcer les situations dangereuses.<\/p>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n<\/div>\n\n\n\n<div id=\"the-issue\" class=\"wp-block-ob-blocks-anchor-point\"><\/div>\n\n\n\n<div class=\"wp-block-columns is-layout-flex wp-container-core-columns-is-layout-28f84493 wp-block-columns-is-layout-flex\">\n<div class=\"wp-block-column gradient-top purple is-layout-flow wp-block-column-is-layout-flow\" style=\"flex-basis:100%\">\n<h3 class=\"wp-block-heading has-black-color has-text-color\" id=\"TheChallenge\"><img decoding=\"async\" class=\"wp-image-240812\" style=\"width: 12px;height: 24px;margin-right: 10px;position: relative;top: -2px\" src=\"https:\/\/fsc-ccf.ca\/wp-content\/uploads\/2024\/07\/Vector-2.svg\" alt=\"\"> L\u2019enjeu<\/h3>\n\n\n\n<div class=\"wp-block-group flex-print is-layout-constrained wp-block-group-is-layout-constrained\">\n<div class=\"wp-block-group is-layout-constrained wp-block-group-is-layout-constrained\">\n<p>La formation des pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne porte traditionnellement sur les soins de base et les activit\u00e9s de la vie quotidienne, comme la toilette, mais ne s\u2019int\u00e9resse pas suffisamment aux troubles de la communication et aux comportements r\u00e9actifs qui sont souvent manifest\u00e9s par les personnes atteintes de d\u00e9mence. Du fait de ces insuffisances, les soins sont souvent plus ax\u00e9s sur les t\u00e2ches que sur la personne, si bien qu\u2019il peut arriver que des personnes atteintes de d\u00e9mence puissent ressentir de la d\u00e9tresse ou refuser des soins et, dans les cas graves, qu\u2019elles fassent l\u2019objet de mauvais traitements et que des pratiques standard ne soient pas respect\u00e9es, comme cela a \u00e9t\u00e9 observ\u00e9 pendant la pand\u00e9mie de COVID-19.<\/p>\n\n\n\n<p>La pand\u00e9mie a mis en lumi\u00e8re, d\u2019une part, le r\u00f4le essentiel que jouaient les pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne dans les \u00e9quipes de soins et, d\u2019autre part, les lourdes cons\u00e9quences d\u2019une formation insuffisante sur la qualit\u00e9 des soins aux personnes atteintes de d\u00e9mence et sur le bien-\u00eatre des prestataires de soins. Les pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne se sont vu confier des responsabilit\u00e9s qui relevaient auparavant des professionnels de la sant\u00e9 r\u00e9glement\u00e9s, mais leur formation n\u2019a pas \u00e9t\u00e9 actualis\u00e9e, ce qui s\u2019est traduit par une pression accrue et, en d\u00e9finitive, par des cas d\u2019\u00e9puisement professionnel et des taux de rotation \u00e9lev\u00e9s. Cette situation a \u00e9t\u00e9 aggrav\u00e9e par les probl\u00e8mes de recrutement et de maintien en poste, qui tiennent \u00e0 la nature exigeante des soins aux personnes atteintes de d\u00e9mence, aux politiques de sortie anticip\u00e9e des h\u00f4pitaux ainsi qu\u2019\u00e0 la r\u00e9mun\u00e9ration insuffisante et au manque de reconnaissance professionnelle des pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne.<\/p>\n\n\n\n<p>Les initiatives men\u00e9es avant le projet pour combler ces lacunes en mati\u00e8re de formation \u00e9taient rares et n\u2019\u00e9taient pas suffisamment compl\u00e8tes pour r\u00e9pondre aux besoins complexes des personnes atteintes de d\u00e9mence ou pour doter les pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne des comp\u00e9tences et de la confiance n\u00e9cessaires. Les programmes de formation propos\u00e9s ne pr\u00e9paraient pas bien les pr\u00e9pos\u00e9s \u00e0 g\u00e9rer les probl\u00e8mes de communication complexes et les difficult\u00e9s comportementales des personnes atteintes de d\u00e9mence. Les groupes cibles du projet \u00e9taient les pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne travaillant dans des foyers de soins de longue dur\u00e9e ou \u00e0 domicile ainsi que leurs responsables. Le projet visait \u00e0 doter ces travailleurs de la sant\u00e9 de premi\u00e8re ligne des aptitudes en communication ax\u00e9e sur la personne indispensables pour am\u00e9liorer la prise en charge des personnes atteintes de d\u00e9mence. L\u2019utilisation de la technologie de r\u00e9alit\u00e9 virtuelle a \u00e9t\u00e9 choisie pour surmonter les limites g\u00e9ographiques et offrir une exp\u00e9rience de formation \u00e9volutive et immersive. Le sigle EPIC signifie Environnement, Personne (communication ax\u00e9e sur la), Importance de soi-m\u00eame et Capacit\u00e9s et pr\u00e9f\u00e9rences du client.<\/p>\n\n\n\n<p>Le programme Be EPIC-VR s\u2019inspirait d\u2019un premier programme de formation en pr\u00e9sentiel (programme Be EPIC). Le projet consistait \u00e0 concevoir, \u00e0 ex\u00e9cuter et \u00e0 mettre \u00e0 l\u2019essai le programme en plusieurs phases. La formation a \u00e9t\u00e9 dispens\u00e9e \u00e0 l\u2019aide d\u2019une solution de r\u00e9alit\u00e9 virtuelle de pointe, qui comprenait des modules interactifs et permettait d\u2019interagir en temps r\u00e9el avec des avatars repr\u00e9sentant des personnes atteintes de d\u00e9mence. Dans un premier temps, des tests b\u00eata ont \u00e9t\u00e9 men\u00e9s aupr\u00e8s de sept pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne qui avaient particip\u00e9 au projet initial en pr\u00e9sentiel. Des tests pilotes ont ensuite \u00e9t\u00e9 r\u00e9alis\u00e9s aupr\u00e8s de huit responsables de pr\u00e9pos\u00e9s, et une \u00e9tude de faisabilit\u00e9 a \u00e9t\u00e9 men\u00e9e avec la participation de huit pr\u00e9pos\u00e9s. La phase finale a consist\u00e9 \u00e0 proposer la formation \u00e0 44&nbsp;pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne et professionnels occupant des fonctions similaires, en recourant \u00e0 deux groupes parall\u00e8les (un groupe de formation imm\u00e9diate et un groupe t\u00e9moin sur liste d\u2019attente).<\/p>\n\n\n\n<p>Les questions de recherche qui ont guid\u00e9 le projet visaient \u00e0 \u00e9valuer l\u2019efficacit\u00e9 du programme de formation Be EPIC-VR, \u00e0 d\u00e9terminer les facteurs influant sur la mise en \u0153uvre du programme dans les foyers de soins de longue dur\u00e9e ou \u00e0 domicile, et \u00e0 \u00e9valuer les r\u00e9percussions de la r\u00e9alit\u00e9 virtuelle sur les r\u00e9sultats de la formation. Cette formation faisait appel \u00e0 la r\u00e9alit\u00e9 virtuelle et comportait des simulations et des interactions r\u00e9alistes avec des avatars repr\u00e9sentant des personnes atteintes de d\u00e9mence. L\u2019un des principaux r\u00e9sultats a \u00e9t\u00e9 le niveau \u00e9lev\u00e9 de satisfaction des participants. Pr\u00e8s de 98&nbsp;% des pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne ont jug\u00e9 la formation utile et 79&nbsp;% l\u2019ont recommand\u00e9e \u00e0 d\u2019autres personnes avant la fin du projet. Ce degr\u00e9 d\u2019approbation t\u00e9moigne de l\u2019utilit\u00e9 per\u00e7ue de la formation et des possibilit\u00e9s d\u2019application imm\u00e9diate dans des contextes r\u00e9els.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>La crise des ressources humaines a entra\u00een\u00e9 des difficult\u00e9s de recrutement<\/strong><br>Le projet s\u2019est heurt\u00e9 \u00e0 de nombreux probl\u00e8mes, notamment en ce qui concerne le recrutement de pr\u00e9pos\u00e9s aux services de soutien \u00e0 la personne. La crise des ressources humaines en sant\u00e9, accentu\u00e9e par la pand\u00e9mie de COVID-19, a entra\u00een\u00e9 des retards et une participation plus faible que pr\u00e9vu. Malgr\u00e9 ces obstacles, le projet a atteint 92&nbsp;% de l\u2019objectif fix\u00e9 en mati\u00e8re de recrutement, preuve de sa r\u00e9silience et de sa capacit\u00e9 d\u2019adaptation.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>Les responsables ont jou\u00e9 un r\u00f4le crucial dans la mise en \u0153uvre du programme Be EPIC-VR<\/strong><br>Leur mobilisation a permis d\u2019aligner la formation sur les objectifs organisationnels et de faciliter l\u2019adoption de nouvelles pratiques. Les responsables ont dit que la formation \u00e9tait adapt\u00e9e \u00e0 leurs besoins en mati\u00e8re de gestion des comportements r\u00e9actifs et des troubles de communication des personnes atteintes de d\u00e9mence. Ils \u00e9taient satisfaits de la solution de r\u00e9alit\u00e9 virtuelle, qui offrait un espace s\u00fbr leur permettant de s\u2019entra\u00eener et de perfectionner leurs comp\u00e9tences sans les pressions imm\u00e9diates du milieu de travail.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>Il est utile de renforcer l\u2019\u00e9quit\u00e9, la diversit\u00e9, l\u2019inclusion et la r\u00e9conciliation<\/strong><br>Le contenu de la formation a \u00e9t\u00e9 notamment salu\u00e9 pour sa sensibilit\u00e9 culturelle et linguistique, \u00e9l\u00e9ment indispensable pour r\u00e9pondre aux besoins divers du personnel et des personnes atteintes de d\u00e9mence prises en charge. Cet aspect de la formation a \u00e9t\u00e9 particuli\u00e8rement utile aux membres du personnel dont la langue maternelle n\u2019\u00e9tait pas l\u2019anglais, car ils ont ainsi pu aff\u00fbter leurs aptitudes en communication et acqu\u00e9rir une plus grande confiance en soi lors de la prestation de soins.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>Les limites de l\u2019intelligence artificielle d\u00e9\u00e7oivent certains participants<\/strong><br>Malgr\u00e9 les succ\u00e8s obtenus, des possibilit\u00e9s d\u2019am\u00e9lioration ont \u00e9t\u00e9 relev\u00e9es. Les limites de l\u2019intelligence artificielle conversationnelle dans les simulations de r\u00e9alit\u00e9 virtuelle ont parfois suscit\u00e9 un certain m\u00e9contentement chez les participants qui avaient des attentes \u00e9lev\u00e9es quant aux capacit\u00e9s de la technologie. Le renforcement \u00e0 venir des capacit\u00e9s de l\u2019intelligence artificielle en mati\u00e8re de gestion des interactions en langage naturel pourrait am\u00e9liorer le r\u00e9alisme et l\u2019efficacit\u00e9 de la formation.<\/p>\n\n\n\n<p><\/p>\n\n\n\n<p><strong>Ce mod\u00e8le pourrait \u00eatre reproduit et transpos\u00e9 \u00e0 plus grande \u00e9chelle<\/strong><br>Fort de son succ\u00e8s, le mod\u00e8le de formation en r\u00e9alit\u00e9 virtuelle pourrait \u00eatre en effet d\u00e9velopp\u00e9 et int\u00e9gr\u00e9 dans les exigences nationales de formation, ce qui influerait sur les futures politiques et pratiques en mati\u00e8re de soins de sant\u00e9. Cela pourrait aboutir \u00e0 une am\u00e9lioration globale de la qualit\u00e9 des soins fournis aux personnes atteintes de d\u00e9mence, ce qui renforce la n\u00e9cessit\u00e9 d\u2019adopter des strat\u00e9gies de communication adapt\u00e9es aux ant\u00e9c\u00e9dents, aux pr\u00e9f\u00e9rences et aux capacit\u00e9s de la personne. En janvier 2024, Sant\u00e9 Ontario Ouest a adopt\u00e9 le programme Be EPIC-VR.
